Patient Forms

Paperwork can sometimes be overwhelming at registration time. To help facilitate the process, we have provided several forms on this website that you can print out and complete before your visit. Please remember to bring them along for your appointment. This will help reduce the time you spend in the waiting room.

New Patients

We welcome you to Center for Obstetrics & Gynecology, and look forward to a long, healthy relationship. Before your first visit, there are some things we’d like you to know.

Please bring your insurance card with you to all appointments, and understand that we are required to collect your copay at the front window before you are seen for your appointment. This procedure is part of our contractual obligation with your insurance provider, and we do not have the authority to change it, or make exceptions.

Please understand that our knowledge of your insurance benefits is general. We suggest that you read about the benefits your plan offers before your first appointment with us and call your insurance provider directly with any questions.

When you call the Center with common questions or minor problems, please be ready with as much information as possible to help us. Our qualified Medical Assistants and Nurses can help with advice or treatment options according to our guidelines. If they are uncertain, they will consult with a physician and return your call. Should your situation be an emergency, you will be asked to go directly to Women and Infant’s Hospital for evaluation.

No Show Policy

We realize that some things happen that are out of our control. However, effective March 2012, we have a new policy for missed appointments at Center for OB/GYN. There is now a $25 fee for a missed appointment and a $50 fee for a missed office procedure.

A missed appointment is time our providers could be spending with another patient who is on our waiting list to come into our office.

If you need to reschedule or cancel an appointment, please give us at least 24 hour notice. If you do not give us 24 hour notice, or if you do not show up for a scheduled appointment, you may be charged a $25 fee for a missed office visit or $50 for a missed procedure or testing appointment.
